FOR MEN'S OVER 35 LEAGUE: Please complete the
2009 Winter Indoor Registration Form to be eligible to play in
the upcoming season. FOR OPEN RECREATIONAL LEAGUE: Please complete the
2009 Winter Indoor Registration Form to be eligible to play in
the upcoming season. For getting registered you can either meet us in person
at the Metrosports facility during the following times: Monday January 5 -
7:00pm-7:45pm Tuesday January 6 - 9:00pm-10:00pm or mail the
application and cheque to the address at the bottom of the registration form.
You can also arrange to pay via email money transfer - please email Jonathan at
stmikesoccer@rogers.com to set this
up. We will be having additional registration drop offs on Monday and Tuesday
during January - check website for times as these will change from week to week. Applications will be processed immediately upon receiving payment.
- The cost of registration for the 2009 Winter Indoor session is as follows -
$160/player (League provides team/player with t-shirt uniform) or $150/player
(Team provides own uniform).
- To register an entire team spot, a deposit of 5 players must be fully
paid up.
- Once a team deposit of $750 (uniforms not required) or $800 (uniforms
required) has been received, the remainder of team fees are due by February
3, 2009
- There will be a $10 per player late fee for any payments made after
February 4, 2009
- Teams are also required to submit a team bond of $125 that is due
February 17, 2009
- TEAM DEPOSITS ARE NON-REFUNDABLE - NO EXCEPTIONS
- Payment after February 3, 2009 will only be accepted in the
form of a certified cheque, money order or cash. There is a $25.00 charge
for cheques that are returned as NSF.
- There is a cancellation fee of $25.00 after February 3, 2009.
- There is a cancellation fee of $10 for any processed application forms
- No refunds will be provided after February 23, 2009
- After February 24, 2009 there will be a charge of $25 for any switching of
players.
- In order to be fully registered for the 2009 Winter indoor soccer season, this
form must be properly signed and a cheque, money order or cash for the full
amount must accompany this form
- Any modification to the wording of this registration form will result in
an unprocessed application.
